What Should I Bring? When making an appointment, you have two options for filling out your new patient intake forms. You can print them from the "New Patient" tab and fill them out before your first visit. The second option is to come in 10 minutes before your initial visit and we would be happy to provide the intake forms for you. How Long Will It Take? We set aside one hour for the initial history, exam, treatment, and paperwork. It may actually take less time than this, but is dependent on the patient, conditions, and varied treatments. Please be 5-10 minutes early to assure that all intake forms are completed before your visit.
Forms of Payment Horace Family Chiropractic is an in-network provider for Blue Cross/Blue Shield, Sanford Health Plan, Medicaid and Medicare. We submit to ALL insurances whether in network or not, and have found that some out of network insurance plans carry the same benefits. Whether using insurance or not, Horace Family Chiropractic accepts cash, checks, Visa, and Mastercard and will work to accommodate all budgets.
How much will it cost? Cost is dependent on individual insurance plan, initial visit with exam is approximately $85. Self-pay option is $50 per visit if not using insurance.
