Whether you've been introduced to chiropractic care in the past or not, we want every person who walks in our doors to feel welcomed and comfortable. Horace Family Chiropractic wants you to feel "at home" and encourages entire families (yes, little ones too!) to come with to your visit. After all, entire families will reap the positive benefits of your care, while having an opportunity to learn and ask questions. We welcome questions and comments at all times. Your initial consultation will consist of a review of history, exam, and therapies or treatment. After this, we will ask some questions to get an idea about your overall health and what you feel is the reason for you visit. Next, an exam will be performed. In most cases, Dr. Schultz will have collected all information we need to make a proper diagnosis and treatment plan, and you will be adjusted and treated on the same day.

What Should I Bring? When making an appointment, you have two options for filling out your new patient intake forms. You can print them from the "New Patient" tab and fill them out before your first visit. The second option is to come in 15 minutes before your initial visit and we would be happy to provide the intake forms for you. How Long Will It Take? We set aside one hour for the initial history, exam, treatment, and paperwork. It may actually take less time than this, but is dependent on the patient, conditions, and varied treatments. Please be 5-10 minutes early to assure that all intake forms are completed before your visit.
Forms of Payment Horace Family Chiropractic is an in-network provider for Blue Cross/Blue Shield, Sanford Health Plan, Medicaid and Medicare. We submit to ALL insurances whether in network or not, and have found that some out of network insurance plans carry the same benefits. We would be happy to help you find out your benefits if you call with your policy number. Whether using insurance or not, Horace Family Chiropractic accepts cash, checks, Visa, and Mastercard and will work to accommodate all budgets.
